For NYC we only use one form to help us account for events, fairs, workshop and special program numbers that are not captured for EFNEP or FMNP. This form is used by supervisors and with interns, and on occasion for CEs who may be involved in a non EFNEP reportable event or training. It is very easy to forget to report our work!
